This screening is an assessment use to obtain vital informational to check for COVID-19 symptoms as outlined by the Centers for Disease Control (CDC).  It is important that we stay safe and help prevent the spread of the Corona-virus.  All host companion care home providers and residential staff are required to us this assessment when visitor enter the home or facility.

Based on answers, the tool will be provide to HHSC and use internally to ensure compliance to HHSC an CD policies.
